OBESLIM

Orlistat is a drug designed to treat obesity. Its primary function is preventing the absorption of fats from the human diet, thereby reducing caloric intake.

Mechanism of Action:

OBESLIM is the reversible inhibitor of lipases. It exerts its therapeutic activity in the lumen of the stomach & small intestine by forming a covalent bond with the active serine residue site of gastric and pancreatic lipases. This results in unavailability of enzymes to hydrolyze dietary fat in the form of triglycerides into absorbable free fatty acids and monoglycerides. Thus, the fat from the diet is excreted undigested and not absorbed by the body.

Indications:
  • Obesity management including weight loss
  • Weight maintenance when used in conjunction with a reduced-calorie diet
  • Indicated to reduce the risk for weight regain after prior weight loss.
CRITERIA FOR THE USE OF ORLISTAT:
  • Patient must have a BMI in excess of 30, or a BMI in excess of 28 which additional risk factors, such as Type 2 diabetes, high cholesterol or hypertension.
  • They should be obese (a BMI of 30 or more) or overweight (a BMI of 27 or more with other risk factors including high cholesterol or type II diabetes).
Dosage:

Route of administration: Oral

12 years or older: 120 mg orally three times a day with each main meal containing fat.

The dose may be taken during the meal or within 1 hour of completing the meal.

 

DIET: You should evenly divide your daily intake of fat, carbohydrates, and protein over 3 main meals.

Pharmacokinetics:

Absorption: Systemic absorption of Orlistat is minimal; however systemic absorption of the drug is not needed for activity.

Protein binding: Approximately 99%

Metabolism: In the GI tract

Route of elimination: Fecal

Half-life: 1-2 hours

Adverse Effects:
  • Changes in bowel function frequently occur
  • Fatty/oily stool
  • Oily spotting
  • Intestinal gas with discharge
  • A feeling of needing to have a bowel movement right away
  • Increased number of bowel movements, or poor bowel control
Contraindications:

OBESLIM is contraindicated in:

  • Malabsorption
  • Hypersensitivity to orlistat
  • Reduced gallbladder function (e.g. after cholecystectomy)
  • Pregnancy and breastfeeding
Precautions:
  • Obstructed bile duct
  • Impaired liver function
  • Pancreatic disease
Terminologies:

Malabsorption: It is a state arising from abnormality in absorption of food nutrients across the gastrointestinal (GI) tract.
